Whānau Ora
Whānau Ora is a culturally-based, and whānau-centred approach to wellbeing focused on whānau (family group) as a whole, as the decision-makers who determine their goals and aspirations.
Building on the strengths and capabilities of whānau and wrapping the necessary services and support around them to get better outcomes and create positive changes. In areas such as health, education, housing, employment, improved standards of living and cultural identity.
Whānau are supported to fully realise the confidence, mana and the belief in self, whānau and community.

Healthy Families East Cape
Healthy Families East Cape is a team of social innovators and indigenous systems thinkers working to encourage a united effort for better health and wellbeing outcomes. As part of a large-scale initiative, Healthy Families NZ, which brings communities and community leadership together to improve people’s health and wellbeing where they live, learn, work, and play. The initiative is focused on community-led innovation and systems-wide prevention solutions, with a strong emphasis on equity, improving health for Māori, and reducing inequities for groups at increased risk of preventable chronic diseases.
Taking a community approach to complex issues within systems acknowledges that the most effective and sustainable solutions to health and wellbeing challenges are best driven by the people who are most affected.

(ICM) is a navigation and coordination service designed to support individuals with long-term conditions or complex health needs. Your Kaimanaaki will assist you in moving through the health and social services system, ensuring closer integration with your general practice.
